This form is primarily designed for teachers to request participation on behalf of their classes: single A(S)-level student applications will be considered, but please check with your teacher as to whether a group application from your school will be made before applying!

Please provide as accurate an estimate of the number of attendees as possible, as space is limited in the venue.
